PERPANJANG IJIN HAJI KHUSUS (PIHK)

Semoga Allah SWT Yang Maha Agung senantiasa melimpahkan rahmat, hidayah & inayah-Nya serta membimbing kita ke jalan yang lurus dan diridhai-Nya. Aamin Ya Rabbal Alamiin.

Semoga Allah SWT senantiasa mengaruniakan jalan untuk kita agar dapat berkunjung dan beribadah menyempurnakan rukun Islam kita, yaitu menunaikan ibadah Haji sebagai rukun Islan yang kelima.

Berikut kami sampaikan beberapa persyaratan untuk perpanjangan ijin operasional Haji Khusus (PIHK) sebagai berikut :

1. Surat Permohonan yang ditujukan kepada Direktur Jenderal Penyelenggaraan Haji dan Umrah.

2. FC SK Umrah yang masih berlaku

3. FC SK Haji yang masih berlaku

4. FC NPWP Perusahaan

5. FC NPWP Pimpinan Perusahaan

6. FC Akte Pendirian Perusahaan serta Akte Perubahannya yang telah disyahkan oleh Kementerian Hukum dan Ham.

7. FC Daftar Ulang (Rekomendasi) atau TDUP dari Kementerian Pariwisata Wilayah setempat.

8. Struktur Organisasi Perusahaan (ditanda tangani dan distampel)

9. FC Laporan Keuangan Perusahaan yang telah diaudit oleh Akuntan Publik

10. FC Bukti telah memberangkatkan jamaah haji minimal 200 (dua ratus) orang selama 3 (tiga) tahun berturut turut.

11. FC Bank Garansi sebesar Rp. 500.000.000,- (lima ratus juta rupiah) dan berlaku minimal 4 (empat) tahun.

12. Bukti Akreditasi dari Kementerian Agama Wilayah setempat

13. Surat Rekomendasi dari Kementerian Agama Wilayah setempat

14. Surat Tugas, apabila menugaskan kepada karyawan yang ditunjuk

15, Surat Pernyataan dari Pimpinan.
